Effective care navigation helps people understand what options are available to them, enabling them to access the most appropriate service to meet their health and social care needs.
With the recent news that the NHS is funding one person per practice, what about training for the rest of your team? It’s important that the people handling patient enquiries are trained to understand care navigation. That’s where we can help.
Practice Index offer a range of care navigation courses to meet your needs.
Complete Care Navigation Programme
Available through Practice Index Training, this half-day programme will help to develop the relevant skills of non-clinical staff in general practice, supporting them in enhancing the care pathway.
Care Navigation (Active Signposting)
These training sessions help staff understand the principles of care navigation, the positive impact care navigation can have, and how a directory of services can help patients access the right care in a timely manner.
Care Navigation Training Course
Split over three modules, this course enables delegates to combine eLearning and a trainer-led virtual session and covers all of the following:
- An introduction to Care Navigation – A short eLearning course, accessible in the HUB.
- Care Navigation Virtual Training – A 90-minute training session, delivered virtually using Zoom or MS Teams.
- Care Navigation and Correspondence Management Masterclass, – An eLearning course which takes approximately 2.5 hours to complete.
Our popular Care Navigation Bundle consists of three Virtual Training sessions:
Care navigation ensures that patients are seen by the right person, in the right place and at the right time. Furthermore, good care navigation can help patients by offering continuity of care, when preferred, whilst also supporting the Making Every Contact Count (MECC) approach to support patients.
Care navigation training is a good way to develop your non-clinical team and to enhance care navigation within your practice, whilst improving population health.
