During this session delegates will learn what organisations can do to ensure patient need is met. We will draw upon the link between meeting patients’ needs and supporting patients adopt a self-management approach “helping people gain and use the knowledge, skills, tools and confidence to become active participants in their care so that they can reach their self-identified health and wellbeing goals”.

  • Learn how to motivate patients in using engaging coaching techniques
  • Understand what is needed to maintain patient dignity and treat patients with respect 
  • Be able to identify patients’ care and support needs by asking the question “what matters to you?”

  • Gain an overview of the self-care skills which care-coordinators can share with patients
