During this session delegates will learn what organisations can do to ensure patient need is met. We will draw upon the link between meeting patients’ needs and supporting patients adopt a self-management approach “helping people gain and use the knowledge, skills, tools and confidence to become active participants in their care so that they can reach their self-identified health and wellbeing goals”.
