Therapy review services
Interface pharmacists can support primary care to deliver long-term condition reviews, including those in atrial fibrillation, asthma, bone health, COPD, diabetes and heart failure.
Medicines optimisation services form the basis of effective prescribing strategies in primary care and as such, they are fundamental elements of the therapy review services Interface pharmacists can deliver.
Patient clinics
Many of these therapy review services can incorporate patient clinics, which can be delivered face to face or remotely as currently required due to the COVID-19 pandemic. Pharmacist-led patient clinics can include:
- Assessment of patient symptoms using validated screening questionnaires
- Counselling of patients to support improved understanding and adherence to prescribed treatment
- Facilitate necessary treatment interventions to support the management of patients with long term conditions (in accordance with clinical guidelines and under practice direction)
- Ensure patients are compliant with their treatment regimens alongside improved use of medical devices, such as inhalers and spacers
QOF Disease Prevalence Service
Interface pharmacists can identify patients missing from clinical registers to increase QOF prevalence and income. These patients are often being treated but are missing from the QOF registers due to coding errors. This means that practices are not receiving the correct level of funding. Without proper register maintenance, patient care may be impacted with patients missing annual reviews for example.
Our pharmacists have developed a bespoke set of searches to support practices and PCNs with their QOF achievement. On average, we identify £1,800 of additional income per 1,000 list size. This is in addition to improving QOF prevalence and disease register validation.

Now more than ever, it is important to have accurate disease registers to ensure patients are receiving the correct guidance and care.
Heart Failure Plus
The Heart Failure Plus Service is an additional module of our QOF Disease Prevalence Service. In delivering QOF services to over 1,000 practices we recognised a need for additional support within Heart Failure.
There is a poor prognosis within the Heart Failure domain. The National HF Audit and NICE project that LVSD is the cause of HF in 50-70% of cases, yet official QOF data shows it’s only coded in 30% of patients with Heart Failure.
The Heart Failure Plus tool closes this gap, supporting practices by further interrogating the Heart Failure QOF domain, identifying and adding appropriate patients to the register, enabling a more targeted management of this vulnerable group. By looking deeper into the data we are refining the diagnosis of all patients within the domain, including those with LVSD and LVDD.
